How to book us
We need to process three documents with you in order to give a Jazzistry performance.
(1) Booking Form, (2) Agreement and (3) Checklist.
1. FILL IT OUT THE BOOKING FORM COMPLETELY
§ Print Booking Form.
§ Email or fax or mail your form to us as soon as possible. The information
on it is critical. The sooner we get it, the better your chance of getting your
choice of dates*.
§ Please indicate a contact person with whom Jazzistry will communicate
about everything. Let us know if the contact will be different on the day of
the performance.
§ Info for the contact person and the person providing payment must be
on the Booking Form.
*We urge you to select your dates with care to avoid a $200 cancellation fee.
Many administrative hours are required to move the date of a gig once it has
been set and blocked out on the calendars of all the musicians and assisting
crew. Consider that other schools might want your date and be unable to book
because of your reservation.
Jazzistry will contact you and discuss all the details after receiving your
Booking Form -price, preferred length of performance, size of audience, ties
to curriculum or other school themes, performance space, payment, etc.
2. REVIEW AND SIGN YOUR AGREEMENT
§ You will receive an Agreement drawn up from the information on your booking
form and our follow up conversation.
§ Your signed Agreement is needed to officially reserve your date. Once
we have your signed Agreement, you are guaranteed the time and the day indicated.
3. USE YOUR CHECKLIST
§ A useful preparation Checklist comes with the Agreement.
§ Addressing everything listed will insure a smooth and timely performance.

TERMS
§ The booking organization is responsible for providing a tuned piano and
having it on location the day of the performance.
§ All parties will make every effort to honor the times indicated on the
contract.
§ Payment in full by check is expected on the day of the performance.
§ A cancellation fee of $200 will be charged if a date on a signed contract
is changed and the reservation prevented another party from booking that date.
